Build a small evidence index
Create one row in your own notes for each claimed condition. Record the document name, date, page, and what it shows. A diagnosis, an event during service, and a connection between them answer different questions; one record may not establish all three.
For an original service-connected claim, VA generally looks for a current disability, an in-service event or illness, and a link. Presumptive conditions and different types of service have their own requirements. Use VA’s evidence page for the claim type that actually applies to you.
Collect records and identify missing ones
Gather separation documents, service treatment records, and relevant medical records, or provide VA the information and permission needed to obtain them. Keep the facility name and treatment dates alongside each missing record so a request is specific.
Separate a record you already possess from one you have requested. A practical tracking note might say: ‘Orthopedics visit, March 2025; requested from clinic; not yet received.’ Do not describe a request as evidence already in VA’s file.
Use statements for firsthand observations
A personal or buddy statement can explain events and symptoms someone actually observed. Name the approximate dates, describe the observation, and distinguish personal knowledge from something another person told you. VA accepts lay evidence and provides forms for it.
Keep a working copy of your index and copies of the documents you send. Before submitting, compare your package with the application’s current instructions and any specific evidence request from VA.
